For Mentor/Mentee, please have only one person fill out the form, we will be sure to apply credits to both profiles (eg. Mentor apply, CE will be applied to both Mentor and Mentee profile)
PLEASE NOTE: Mentor and Mentee must both be current IAPF members for CE consideration.
Application for IAPF Continuing Education Credits for MENTORING
For long term application, each Mentor/Mentee relationship, MAX 40 CE per calendar year, please report by October 31 of each year.
